Use this form to submit this years Beautiful Faces of Autism for our picture campaign. All fields must be completed before submitting.
Please READ before filling out the form!
Submission Instructions:
- Only those on the Autism Spectrum should be submitted for this campaign.
- Only the person on the Autism Spectrum should be in the photo – One person only. (Sorry mom, dad and other family – this is their time to shine)
- Please make sure it is the photo you want to be shown. There will be no resubmissions allowed this year. First photo received will be the photo shown. Some cropping of the photo on our end may be necessary.
- If you have more than one loved one on the spectrum, you must submit each seperately. (Submit one then come back to the form to submit another)
- Please be sure to READ the consent area before checking the box and submitting – This campaign is also a fundraiser for the Autism And Us community.
- Deadline for submissions will be April 26th – NO EXCEPTIONS
- Lastly, please be patient. All photos will be shared in the order in which they are received.
